Mental health and community support services, including case management, supportive housing, short-term residential beds, day programs, court and justice related services, Assertive Community Treatment (ACT) Team, homeless services

- Eligibility
- Individuals 16-65 years living with serious mental illness, geriatric mental health conditions, acquired brain injury, developmental disabilities and/or dual diagnosis; individuals experiencing homelessness; varies by program, call for details
