Geriatric Outreach Team
  • in-home assessment of homebound seniors living in the community who have complex problems related to health, day-to-day functioning and social concerns
  • care coordination and follow up
Geriatric Outpatient Clinic: assessment from geriatrician working with interdisciplinary team and with patient's primary care provider

W.P. Scott Geriatric Day Hospital: individualized treatment plan based on patient's own goals and abilities

Falls Prevention Program
  • consultation with geriatrician, nurse, physiotherapist and occupational therapist
  • group exercise program designed to improve strength, balance and gait
  • home safety and home exercise information session
Sunnybrook is a member of the Regional Geriatric Program of Toronto (RGP)
Eligibility
Seniors experiencing complex difficulties associated with aging that interfere with functioning, health and well-being
