Touching Base Program


Description
Transitional support program includes ongoing, confidential telephone reassurance and support, 3-6 months during client crisis or transition, such as hospital to home, between services, or from crisis

Program includes:
  • medication reminders, safety checks, emotional support
  • assistance with anxiety, stress, depression and sadness, coping with work and family life, relationships, abuse, isolation and loneliness, loss and grief, parenting, divorce, settlement and integration, relocation, work issues and job loss, sexual orientation and coming out issues
Eligibility
Youth and young adults 16 years and older, who are experiencing emotional distress due to various life transitions
