- Clients 18 years and older must be eligible for aging at home programs or have a physical disability and/or traumatic brain injury
- clients or their substitute decision maker must be able to direct their service
- other needs must be met within the community's existing health care system
- must be responsible for their rent and living expenses and be accepted by the landlord for tenancy
