Mobile clinic that provides services to people who are experiencing homelessness or who are underhoused

Moss Park Interprofessional Primary Care (IPC) Program

Provides low-barrier primary care to those experiencing homelessness or who are underhoused. Operates every weekday in Moss Park area (please see schedule for times and locations) - staffed by Case Managers, Nurse Practitioners and Community Support Workers.
  • general health assessments, preventative screening and point of care testing
  • referrals to primary care and healthcare providers
  • ongoing primary care including wound care and basic foot care
  • overdose prevention (Naloxone) and harm reduction supplies
  • community navigation, case management
  • preventative care such as PAP tests and colorectal screening
  • sexual health testing including HIV and Hep C testing
  • safer sex and harm reduction supplies
Eligibility
Individuals who face barriers accessing health services, such as those experiencing homelessness or are under-housed; immigrant, refugee and newcomer communities; LGBT2
SQ community members; or people who face barriers connecting with primary care providers
