- clutter removal and falls prevention
Psychogeriatric Resource Consultant
- consultation to primary care clinics, support agencies and caregivers providing service to older adults with cognitive impairment and associated responsive behaviours
Assisted Living Program
- on-site personal care, homemaking, case coordination, 24 hour emergency response
- for eligible residents of designated buildings in Etobicoke, West Woodbridge and Brampton
Friendly Visiting
- phone calls and medication reminders
Ride Connect Transportation
- provides transportation for seniors and clients with disabilities to medical appointments and social activities in Etobicoke, Brampton, Malton and Woodbridge areas
Seniors Intensive Community Support
- case management for seniors living with mental health conditions
- in partnership with Reconnect Mental Health Services and Canadian Mental Health Association - Peel
- Eligibility
- Seniors 65 years and older, some programs seniors 55 years and older
Residency Requirements: Steeles Ave W (north) to Eglinton Ave W (south), Toronto City Limit (west) to Humber River (east)- services available in Etobicoke, Brampton, Malton and West Woodbridge
